Add GitHub Issue template (#749) This prompts the user to include relevant information when reporting issues. I adapted it from the corresponding section of CONTRIBUTING.md See here for details: https://github.com/blog/2111-issue-and-pull-request-templates

ISSUE_TEMPLATE.md: sync format of fill-in fields (#841) Some of the "FILL THIS IN" were wrapped with html comment markup, but most of them weren't. This change makes them all use the same format.
